1. Acceptance of Terms
By scheduling or receiving transportation services from Alpha Care Enterprises, LLC (“Company,” “we,”
“our,” or “us”), you agree to these Terms & Conditions.
2. Nature of Services
Alpha Care Enterprises, LLC provides non-emergency medical transportation (NEMT) services within the
State of Illinois.
Transportation may include:
● Medical appointments
● Dialysis treatments
● Therapy services
● Hospital discharges
● Medicaid-authorized medical trips
● Other approved healthcare destinations
🚨 We do NOT provide emergency medical transportation.
If you are experiencing a medical emergency, call 911 immediately.
3. No Medical Care Disclaimer
Our drivers are not medical professionals unless otherwise specified in writing.
We provide:
● Safe transportation
● Limited mobility assistance
● Curb-to-curb or door-to-door service (as contracted)
We do NOT provide:
● Medical treatment
● Medication administration
● Continuous medical monitoring
● Lifting that exceeds safe transfer limits
Passengers must be medically stable for non-emergency transport.
4. Illinois Regulatory Compliance
Alpha Care Enterprises, LLC operates in accordance with applicable Illinois laws and regulations
governing transportation services, including applicable safety, insurance, and licensing requirements.
For Medicaid recipients, services are subject to approval through the Illinois Department of Healthcare
and Family Services (HFS) or its contracted transportation broker.
5. Client Responsibilities
Clients agree to:
● Provide accurate pickup and drop-off information
● Disclose mobility limitations
● Be ready at scheduled pickup time
● Maintain a safe pickup/drop-off environment
● Ensure minors or vulnerable adults have appropriate supervision when required
Failure to comply may result in service refusal.
6. Scheduling, Cancellations & No-Show Policy
Cancellations
● A minimum of 24 hours’ notice is required for cancellations unless otherwise authorized by a
broker.
● Late cancellations may incur fees for private-pay clients.No-Shows
A client may be considered a No-Show if:
● The client is not ready within 15 minutes of scheduled pickup
● The driver cannot access the pickup location
● The client refuses transport upon arrival
Repeated no-shows may result in suspension of services.
7. Wait Time Policy
Drivers will wait up to 15 minutes unless otherwise arranged. Extended wait time may result in additional
fees for private-pay clients.
8. Payment Terms
Private Pay Clients
● Payment is due prior to or at time of service.
● Late payments may result in service suspension.
Medicaid / Broker-Based Clients
● Trips must be authorized through the appropriate Illinois Medicaid transportation system.
● Clients are responsible for non-covered services or unauthorized trips.
9. Passenger Conduct & Safety
For safety of all parties, the following are prohibited:
● Harassment or abusive behavior
● Illegal substances
● Weapons
● Smoking in vehicles
Alpha Care Enterprises, LLC reserves the right to refuse or terminate service if safety is compromised.
10. Vehicle Safety & InsuranceAll company vehicles:
● Are commercially insured in compliance with Illinois law
● Undergo regular safety inspections
● Are operated by properly licensed drivers
● Meet applicable state transportation safety standards
11. Limitation of Liability
To the fullest extent permitted by Illinois law, Alpha Care Enterprises, LLC shall not be liable for:
● Delays due to traffic, weather, road closures, or mechanical issues
● Missed appointments due to circumstances beyond our control
● Actions of third parties
● Injuries resulting from undisclosed medical conditions
Liability shall not exceed the amount paid for the specific trip in question.
12. Termination of Services
We reserve the right to suspend or terminate services for:
● Non-payment
● Repeated no-shows
● Unsafe conditions
● Violation of these Terms
Outstanding balances remain due upon termination
